Cagrilintide (AM833) is a synthetic, lipidated 37-residue analogue of the pancreatic peptide amylin. Its N-terminus is acylated with icosanedioic acid (C20 diacid) through a γ-glutamyl spacer; the peptide carries a Cys2–Cys7 disulfide bond and a C-terminal proline amide, and its sequence was engineered to reduce the strong amyloid-fibril propensity of native amylin.

Cagrilintide has been characterized as a non-selective agonist of the amylin receptors AMY1R, AMY2R and AMY3R (heterodimers of the calcitonin receptor with receptor activity-modifying proteins, RAMPs) and of the calcitonin receptor (CTR) itself. Comparative pharmacology across receptor binding, activation and regulation endpoints reported a profile distinct from both amylin-selective and calcitonin-like agonists. Cryo-EM structures of Gs-coupled receptor complexes show an amylin-like 'bypass' binding mode with receptor-specific conformational dynamics.

It is used as a reference ligand in calcitonin-family GPCR pharmacology, RAMP–receptor signalling, cAMP and receptor-regulation assays, structural biology of class B GPCRs, and in vitro metabolism studies of lipidated amylin analogues. Lyophilized material is hygroscopic and should be stored desiccated and protected from light.
